Form D book

This is the infamous Form D book (not official without the coffee cup ring on the cover).

The Form D was a written permission slip from the dispatcher in control of a certain portion of track over which we were to travel. It not only gave authorization, it listed any and all restrictions, special conditions or other traffic which would be encountered. This form gave us exclusive rights to that section of track until the limits of travel were completed, at which time the form was canelled by the dispatcher.
